Terms and Conditions       This booking form constitutes a legally binding agreement. If cancellation is confirmed in writing by 4^th April 2008, fees will be refunded, minus administration charge of £50.00. We regret that no refund can be made after that date, for whatever reason, although substitutions are accepted if notified in writing before the event. Conference details will be issued subject to receipt of payment. If you have not received details within 5 days of the 4^th April 2008, please contact the USHA Conference 2008 office.
